Our new Heritage Wellness Center has been designed to offer our exclusive members a one-stop shop for their Senior Wellness Visit, Lab work and other preventive screenings, Glaucoma Screenings, Dilated Eye Exams and other eye consultations with our employed optometrist, Dr. Curtis Hoggarth, Specialist Consultations, Health Education, and Pharmacy Consultation. We believe by offering our members a facility where they can have multiple services provided at the same time and place, we are assisting them in achieving better health and wellness and a better quality of life.

Heritage Wellness Center

The Heritage Wellness Center is a one stop place for all of your preventive screenings and wellness visits. Here we work as a team to preventive quality measurs capture and perform all CMS 5 Star and HEDIS Quality Measures. During a typical visit, you will be asked to participate in a fall risk assessment, a functional status assessment that assesses your ability to care for yourself in your activities of daily living, a pain assessment, depression screening, blood work, a series of cardiac screenings (all non-invasive), glaucoma and dilated eye exams, if indicated, medication review and reconciliation, and an in-depth review of all of your chronic conditions with a provider. Members will leave their exam with the most current and up-to-date labs and medication list as well as an understanding of their chronic conditions.

What are the CMS 5 Star and HEDIS Programs?

The CMS 5 Star and HEDIS Programs are a series of quality measures that doctors and Health Plans are required to perform on their members to ensure they are providing quality care. Why Is Preventive Care Important?

Health screening helps you find out if you have a particular disease or condition even if you feel perfectly well, without any symptoms and/or signs of disease. With preventive screenings, we can intervene early to try to prevent conditions such as diabetes, heart disease, high cholesterol and stroke from developing or progressing. If we can identify patients at risk for such conditions, we can advise them on lifestyle modiļ¬cations and/or recommend medications that may reverse the process and frequently help prevent patients from developing complications or having to start other medications. It is therefore important to get yourself screened even if you feel perfectly healthy. Following the HEDIS or CMS 5 Star Guidelines, along with the advice of your doctor, can help you stay healthy.
